Your GP is committed to ensuring your health is regularly monitored to provide comprehensive management of chronic diseases. Your GP, can support you with treatment of conditions such as diabetes, arthritis, heart conditions, obesity, or respiratory disease.
Your GP, working supported by the BGP nursing team, can provide a comprehensive Annual Care Plan to support you. Annual Care Plans connect you and your GP with specialists and Allied Health professionals to provide a collaborative healthcare and disease management experience.
For eligible Veterans, your GP can also provide specialised care through the Coordinated Veterans Care Program (CVC). The CVC program aims to assist Veterans through tailored care to improve general health and decrease the risk of hospitalisation.
